Integrated Care Partnership

The Integrated Care Partnership (ICP) is a partnership made up of the different providers of health and care in Bolton.

Together they will provide the most appropriate services for local residents in order to help them stay well for longer and in their own homes.

Our Integrated Care Partnership

The Integrated Care Partnership (ICP) brings together primary care, community health services, mental health and adult social care.

We are working together as one team to improve the health and wellbeing of Bolton people and deliver excellent care which is co-ordinated and as close to people’s homes as possible.

The Integrated Care Partnership is made up of the following groups or organisations: Bolton NHS Foundation Trust, Bolton Council, Greater Manchester Mental Health Foundation Trust (GMMH), primary care and the voluntary, community and social enterprise sector (VCSE).

The ICP will focus on the different neighbourhoods within Bolton, working directly with residents to help them build strong, connected and engaged communities that are fully involved in their own health and care services.

From April 2019, the budgets of Bolton Council and Bolton CCG to buy health and care services for adults and older people were pooled to create a total fund of around £160 million.
